An indicator of whether a care coordinator has been assigned.

The role of a care coordinator is to achieve continuity of care and to ensure that care is delivered in a timely, logical and connected fashion so that both medical and personal needs are met.

The assignment of a care coordinator is an indicator of the method of care that is being received.
